To register your child we will need the following:

     Proof of child's residence (copy of lease,rental, sales agreement,

      notarized letter etc.)

      Copy of Birth Certificate

     Immunization record

     Current physical exam record   (including eye exam for kindergarten)

      Completed health and registration packet
